To find Frozen Anguish, you'll need to go to the Eye of Ice in the northern part of Demeniss. Once you're there, you'll have to make the trek to Spire of Frost, a dangerous tower covered in ice behind the mountains.
The area is freezing cold - you might freeze to death. To combat this, wear gear with ice resistance or stock up on food that will protect you from the cold.
